Aptakisic-Tripp School District 102 prioritizes student health, safety, and well-being. In order to address student health and emergency needs, D102 relies on collaboration and communication between home, school, and at times, student medical providers along with the creation of healthcare and emergency action plans. Specifically, plans are required for students who experience: 

1. Severe/life-threatening allergies;

2. Asthma;

3. Diabetes; and/or

4. Seizures

These plans must be developed annually and anytime there is a change in the student’s treatment plan. Please reference the information and forms below and provide the completed and signed forms to the school nurse at your child’s school. Additionally, a School Medication Authorization (SMA) Form must be completed and returned to the school nurse with the completed plan.
