Limited/ Restricted Activity During the School Day
If a student is unable to participate in recess or physical education (PE) due to illness or injury, a written note from the parent or guardian must be submitted to both the school nurse and the PE teacher. This note is valid for up to three days.
If the restriction extends beyond three days, an activity order from a healthcare provider licensed under the Illinois Medical Practice Act is required.
Students who arrive at school with a new cast, splint, or other orthopedic device must also provide an activity order from a licensed healthcare provider.
It is strongly recommended that students returning to school after surgery or hospitalization provide an activity order outlining any necessary restrictions.

Activity orders should include:
The nature of the injury or medical condition
The specific activity the student is restricted from
The duration of the restriction
The expected date the student may resume activity
During the restriction period, students will also be excused from related extracurricular and interscholastic activities.
Students may not return to physical activities until a written medical release is submitted to the school. If needed, the school nurse may contact the healthcare provider to clarify any restrictions.
