Absence or Late Arrival
For your conveniece we have an automated absence phone system that is accessible 24 hours per day. Please call the Absence phone line to report all absences from school: late arrivals, partial day, all day absences, and multiple day absences. Call this line as soon as you are aware your child will not attend school.
Please leave a message with the student’s name, teacher, and reason/length for the absence or late arrival. More information...

COMPLETED HEALTH FORMS MAY BE RETURNED  TO YOUR CHILD'S CURRENT SCHOOL  All student health records transfer from one school level to the next each year.

Injuries/Illnesses at School
The health office will contact you if your child is ill. Children will be sent home if the following symptoms are present: fever of 100 degrees or greater, vomiting, questionable skin rash, discharge from the eyes, or severe cough. Students may not return to school until they are fever-free and have not vomited for 24 hours.  More information...

Medication at School
The nurse may administer medications necessary to maintain a healthy child in school only if an authorization form has been signed by both the physician and the parent. This includes both prescription and over the counter medications. The school health offices do not supply any medication. Only labeled prescription bottles or medication in their original packaging will be accepted.

Asthma inhalers and epipens are the only medications permitted by Illinois law which a student may self carry. Parental written permission is required along with the prescription box for an inhaler to be self carried by a student.

Allergy Guidelines
Please notify your school nurse if your child has any food allergies or dietary restrictions. All life-threatening food allergies coming forward to the school’s attention will initiate an Emergency Action Plan (EAP) personalized for each child. All Individual Health Care Plans (IHP) or 504 Plans will include an EAP for students with life threatening allergies.

Physical Education
All students are expected to participate in physical education classes. If a student is unable to take part for health reasons, a written note from the parent may excuse the student for up to three days from gym. Should it be necessary to be excused for longer than three days, a doctor’s note is required.

Absences or Late Arrivals
All absences and late arrivals must be called in to the school attendance number by a parent prior to a child’s absence. This is an automated phone system available 24 hours a day. Please leave a message if your child will be absent or late.
