Sports Physical Examination for athletic participants at Aptakisic Junior High School (5th through 8th grade)
A current sports physical must be on file in the school nurse's office prior to participating in any after school sport sponsored by AJHS and the IL Elementary School Association (IESA). This includes any try-outs, practices, or pre-season activities. If you have any questions, please contact the school nurse at AJHS or the Athletic Director.
